The book opens with a prologue that situates breastfeeding within a long history of artistic, religious, and scientific representation, tracing how lactation has been understood across cultures and time. The introduction establishes breastmilk insufficiency as a phenomenon that cannot be explained through biology or culture alone, but instead requires an integrated analytical approach. Subsequent sections review earlier researchers and theoretical predecessors, examine cultural beliefs and attitudes toward breastfeeding, and trace the progression from separate biological and cultural explanations toward increasingly integrated, culturally mediated biological perspectives.
Empirical research forms the core of the book. Drawing on fieldwork conducted in Esmeraldas, Ecuador, Herrera explains the development of his research design and the methods used to define and examine breastmilk insufficiency. The analysis brings together bioethnographic data—linking mothers' own explanations and experiences with biological measures—alongside quantitative and qualitative evidence, including interviews, observation, physical measurements of breast milk, and basic body measurements of mothers and infants. The final part of the book develops its central conceptual contribution by situating breastfeeding within a wider "dance" between biology and culture. When many women cease producing milk for their infants, the cause lies not in biological mechanisms alone, but in biology unfolding within socially reconfigured environments shaped and constrained by cultural forces. From this insight, the analysis broadens to human experience more generally, arguing that culture can act directly upon biological processes, reshaping the conditions under which human life unfolds. To name this condition, Herrera introduces the concept of "culbios," proposing a new language for understanding how sustained cultural mediation transforms biological systems and everyday experience. CULBIOS thus designates a mode of human evolution defined by ongoing transformation, in which technologically intensified interconnection shapes experience and opens human life toward what lies beyond the given and the known.
